Do you want to change your name as an adult in Ontario? Look no further than Royal Seal Notary Public in Oshawa. We can help guide you through the process of the Application to Change an Adult’s Name (007-11155E).

Remember, you must be 16 years of age or older and have lived in Ontario for the past 12 months before applying. If you’re 16 or 17 years old, you will need consent from each person who has legal custody over you. If you’re married or have signed a Joint Declaration of Conjugal Relationship, you must also inform your partner/spouse of your name change.

Once your name change is approved, you will receive a change of name certificate and a new birth certificate with your new name. You can then use the certificate to change your name on other personal documents such as your driver’s license or credit cards.

The application must be sworn or affirmed by a commissioner for taking affidavits or a notary public. It is important to ensure that the information provided is true, as it is a criminal offense to swear a false or misleading affidavit.

In the past, sworn statements were required to be signed and witnessed in-person. However, the Ontario Ministry of the Attorney General now permits virtual witnessing of physical signings of sworn statements. Book an appointment with Royal Seal Notary Public to notarize your form with our convenient virtual witnessing service.
